My guess is B, but I don't know the real answer. Can you help me? Thank you!

The correct answer is D, "which led eventually." That's because the clause "Jessica Mitford wrote 'The American Way of Death'" is an independent clause (could be a complete sentence). Therefore, the clause "led eventually to an official investigation of the funeral industry" is NON-essential. Non-essential clauses are attached to sentences with a comma + which.
