IF I HAD KNOWN….....is the correct one .looking at the rule that guide conditonal sentences.for example If i had known i would have broken the pot.had must go with past participle.also it can also be be used as sentence balancing.however,I WISH I HAD KNOW OR KNOWN is a bad english and as no place in the structure of correct usage of conditional sentence.

If i had known i was in bad situation, i would have left  as soon as possible
